Tips and Tricks for Complex DVT & PE Cases Utilizing Computer Assisted Vacuum Thrombectomy (CAVT)
This AVF Venous Confluence session, brings together four interventional specialists to share how computer-assisted vacuum thrombectomy (CAVT) is changing the treatment of venous thromboembolism. Opening with an overview of the Lightning Flash 2.0 system and two complex iliofemoral and caval DVT cases, including profunda femoris thrombectomy and IVUS-guided IVC clearance. Followed by cases involving filter-associated caval thrombosis, acute-on-chronic disease, and IVUS-guided stent reconstruction. Then reviews interim STRIKE-PE trial data, showing reductions in RV/LV ratio and pulmonary artery pressure plus improved quality of life at 90 days, and presents an intermediate-high-risk PE case completed in under 10 minutes of thrombectomy time. Finally, this Confluence session closes with DVT and PE cases and his practical setup for pulmonary thrombectomy.
The panel discussion covers when to stage versus complete a procedure, filter retrieval timing, access site choice and sheath management, prone versus supine positioning, PE in pregnancy, and post-procedure anticoagulation. The shared message: single-session thrombectomy with minimal blood loss is now routine, but chronic disease, IVUS findings and patient-specific factors still guide when to stent and when to stop.
