Day 2 Superficial Venous Session: Ablation Evidence Review
Ablation Technology and Insurance Coverage
An opening talk reviewed strengths and limitations across thermal ablation, mechanochemical ablation, cyanoacrylate closure, and polidocanol microfoam, concluding no single technology solves superficial venous disease but each has situational advantages. A second talk argued history, physical exam, and ultrasound identify appropriate candidates in most cases. Registry-based research found remaining reflux after great saphenous vein treatment was linked to worse clinical scores, below-knee treatment showed less improvement than above-knee treatment, and insurance coverage criteria for ablation varied substantially between single-state and multi-state carriers.
Treatment Comparisons and Managing Thrombosis
Further studies compared microfoam and radiofrequency ablation outcomes, found endovenous ablation safe and effective in patients with prior deep vein thrombosis regardless of anticoagulation status, linked longer treated great saphenous vein segments to smaller improvement, and reported low complication rates in a large ultrasound-guided foam sclerotherapy series. A closing interactive case examined evolving approaches to superficial vein thrombosis, weighing anticoagulation, surgical ligation, and early ablation.
