Day 2 Deep Venous Session: Valve Repair and Stent Outcomes
Valve Reconstruction and Stent Patency Research
Speakers described etiology-based patient selection for deep venous valve reconstruction, including valvuloplasty, valve transplant, and neovalve techniques, alongside midterm results from a bioprosthetic venous valve trial showing sustained improvement in reflux and clinical severity scores. Retrospective studies found that early antiplatelet therapy after iliofemoral stenting was associated with improved one-year patency, that venous stent usage rose sharply after FDA approval with regional and demographic disparities, and that reflux left uncorrected at the time of iliac vein stenting was generally well tolerated clinically.
Age-Related Outcomes and Managing Inflow Challenges
A large single-center study found iliac vein stenting achieved comparable or better symptom resolution in older patients despite greater comorbidity burden. Quick-shot presentations linked lower extremity venous pulsatility to elevated pulmonary artery pressure and tricuspid regurgitation, and showed that many iliac vein stenoses seen supine resolve on standing. An interactive case series closed the session, illustrating techniques for managing inadequate inflow and profunda involvement during venous stent placement.
