International AVF Session: Stents, Nutcracker, and Ulcer Care
Stent Classification and Monitoring Technology
Faculty presented a new classification for venous stent failure, identifying profunda femoris disease as the strongest predictor of reintervention and thrombophilia as a protective factor. A registry analysis linked higher baseline CEAP class to lower odds of patient reported improvement after treatment. Another group validated a sub-bandage pressure sensor, showing digital guidance outperformed visual indicators and that pressure fell substantially within four hours.
Nutcracker Syndrome: Two Approaches
Two speakers contrasted management of Nutcracker syndrome: a multidisciplinary algorithm paired with robotic renal vein transposition achieving high patency, versus open decompression with a polytetrafluoroethylene cuff for hypermobility associated cases, avoiding the complications of venous stenting.
Rethinking Ablation After Thrombosis
A shortened superficial vein thrombosis protocol proved safe with less treatment time. A review of pregnancy related vena cava filters found notable complication rates alongside favorable births. Finally, a study found post thrombotic ulcer patients healed better when a refluxing saphenous vein was preserved rather than ablated, since it can serve as a vital collateral pathway.
