Beyond the Vein: Pelvic PT, Anticoagulation and MPFF
Optimizing the Whole Patient
This session looked past the vein at four areas that shape outcomes in clinic: pelvic floor function, anticoagulation, venoactive drugs and wound care.
Pelvic Physical Therapy
A pelvic physical therapist outlined what an evaluation covers, including lumbar and hip mobility, abdominal and pelvic floor coordination, and breathing mechanics. Patients who bear down whenever they brace are more likely to report pelvic pressure, hemorrhoids or vulvar varicosities. Education, exercise modification, manual and lymphatic therapy and support garments all help while patients wait.
Anticoagulation Decisions
- Avoid direct oral anticoagulants in mechanical valves and antiphospholipid syndrome, though apixaban at standard dose can be used in dialysis.
- Enduring risk factors such as obesity, heart failure or autoimmune disease support extended low dose therapy after a provoked DVT.
- Superficial vein thrombosis longer than five centimeters gets 45 days of low dose anticoagulation, and within three centimeters of the junction it is treated as a DVT.
Beyond Compression
Micronized purified flavonoid fraction has meta-analysis support for symptoms, edema and ulcer healing, and sits alongside compression, pumps, wound management and skin substitutes.
