Rethinking Venous Hemodynamics

A masterclass challenges assumptions about saphenous reflux, showing that reflux volume and vein diameter correlate poorly with calf pump abnormalities: 70 percent of diseased limbs had calf pump dysfunction, but so did 44 percent of limbs without reflux. The talk argues symptom severity depends more on how reflux interacts with the calf pump than on reflux volume alone.

Predictors Of Ablation Outcomes

Registry analyses found obesity and poor preoperative compression compliance, not BMI itself, predict lack of clinical improvement after ablation, while overall ablation failure remained rare. Full-length great saphenous ablation cut symptomatic recurrence roughly fivefold compared to above-knee-only treatment, though transient paresthesia was more common with fuller-length treatment.

Complications And A Challenging Case

Large registry data show varicose vein interventions remain low risk overall, though male sex, larger vein diameter, and anticoagulant use predict recanalization, and limiting treatment to one vein per session reduced severe heat-induced thrombosis after radiofrequency ablation. A closing case describes recurrent bleeding varicosities in a dialysis patient with central venous occlusion, managed with local control alone.
