Crossing And Debulking Tools

Chronically occluded iliocaval veins remain difficult to reopen, and the speaker reviewed useful tools: beveled-needle catheters, power wires and laser devices from the arterial world for stent occlusions, and thrombectomy or debulking systems that remove old material rather than compress it, since compression alone rarely restores the original lumen.

Purpose-Built Stents For Difficult Anatomy

Newer stent designs target specific problems: a hybrid device for the compressive May-Thurner lesion, tapered oblique stents preserving flow from the uninvolved side, and an investigational stent-graft to reconstruct the iliac confluence as one piece rather than two kissing stents that can misalign and threaten patency. A separate catheter delivering dexamethasone around the vein wall is explored to limit fibrosis driving chronic occlusion.

The C-TRACT Trial Nears Completion

All these tools still work mechanically, without addressing the fibrotic biology, and better drugs and comparative-effectiveness data are still needed. The randomized C-TRACT trial, with 220 patients enrolled, compares stenting against no stenting for post-thrombotic iliac vein obstruction with good inflow, tracking severity, quality of life, and safety over two years, with results due soon.
