Bleeding Risk and Basic Science

A new AI model built on 185 predictors from VA health records substantially
outperformed existing bleeding risk scores for patients on prophylactic anticoagulation,
which had performed barely better than chance. Single-cell RNA sequencing in a mouse
thrombosis model showed macrophages differentiating into fibrosis-driving mesenchymal
cells, and found that rosuvastatin reduced vein wall collagen thickness without impairing
thrombus resolution, unlike dexamethasone.

IVC Filter Trends

National registry data showed IVC filter placement has declined by half over the past
decade, with retrieval rates plateauing near 36% and retrieval times lengthening. A
separate registry analysis found Black patients and men had lower odds of filter
retrieval than white patients and women, independent of comorbidities.

Practical Anticoagulation Guidance

A recalibrated Caprini score found higher risk thresholds are needed for vascular
surgery patients, particularly after open aortic repair. Didactic talks reviewed when
thrombophilia testing changes management, and highlighted gaps in DOAC dosing at extremes
of body weight and renal function, cautioning against a one-size-fits-all approach to
extended anticoagulation therapy.
