C-TRACT: Stenting Improves PTS Outcomes at Six Months

In 112 randomized patients with severe post-thrombotic syndrome, iliofemoral venous stenting significantly reduced VCSS and Villalta scores and improved venous specific and general quality of life versus standard care at six months, with similar rates of major bleeding and VTE.

The Jolly Inflow Classification and Patency Outcomes

Patients with normal femoral or profunda inflow, types one through three, had over 80 percent stent patency at 24 months, while poor inflow types four A, four B, and especially five dropped to roughly 60 percent and 30 percent patency respectively.

Vena Cava and RevCore Catheters: Restoring Inflow

The Vena Cava catheter’s outward facing teeth fracture post-thrombotic webs and lumen occupying scar in native veins, while RevCore shaves fibrotic material inside failed stents, with multicenter data showing luminal gain, restored patency, and reduced Villalta scores without major adverse events.

Clinical Pearls: Embolic Protection and Anticoagulation

Faculty recommend routine distal embolic protection when debulking stents, low molecular weight heparin for the first month after inflow restoration before transitioning to a DOAC, and long term anticoagulation for patients needing advanced recanalization.
